Service Description
Community Health Services offers a number of home support services, including support for medical conditions. Registered Dietitians provide assessment, consultation, counselling, and education to support and promote nutritional health. Physiotherapists and occupational therapists optimize independence and safety in the home environment.
They provide assistance for people with medical conditions. These include:
> assessments and referrals for respiratory and other chronic conditions,
> specialized assessment and support services for frail seniors with complex physical and mental health conditions, and
> help with post-hospital care, wound management, and end-of-life support.
Community Health Services offers respite care and assisted bathing services. Case managers maintain the waiting lists for assisted living and long-term care facilities.
They offer help for clients, families, and caregivers with financial matters, abuse/neglect/self-neglect.
Some of these services are offered online or by phone.
